Cohabitation loss of residence and work permit

Question

I am a non-EU citizen living and working in Brussels since 2009. My current partner and I registered our cohabitation in December 2009 and I received my F card in June 2010. Unfortunately the relationship does not work out any more, so we will probably separate and end the cohabitation. I presume that I will automatically lose my residence and work permit with ending the relationship.
1. What is the procedure for applying for the so-called F+ card?
2. Does one have to be in the relationship (with the help of which one received the F card) in order to be able to apply for/receive F+? Does the address with the (former) partner have to be the same?
4. I have heard of the time period of 3 years. Does it mean 3 years from the point of applying for the residence and work permit or 3 years from the point of receiving the F card

Only the IBZ office will be able to give you a definitive answer. I think you have to hold an F card for a minimum of 5 years and remain the partner with whom you were granted the F card to gain full EU treaty rights independent of partner, that's when you should get a F+ card.

Holders of F cards don't require work permits, so you have no work permit to lose.
