How to make tax declaration?

Ok just looking for some advice.

Basically, I was never sent my tax declaration forms. I've been living, working and paying tax in Brussels since 2011. When I contacted the ministry for finance, I was spoken to by an extremely rude woman (how surprising) who informed me that my registration was not completed until 2012, and that's why I wasn't sent my tax declaration.

I was told to order it from the website. However, I can't seem to figure it out. My French is ok but not great. I believe this is the correct page, http://minfin.fgov.be/portail2/fr/themes/declaration/doormat-2012.htm but I'm unsure of what exactly I need to do. The authorities are less than helpful, the commune laughed at me over the phone when I asked for information on the tax declaration. Do I simply need to fill in this document (http://minfin.fgov.be/portail2/downloads/declaration/2012-partie-1-decla...) and return it?

They're making it so difficult that I would normally give up, but I'm owed quite a bit of tax back so it's worth my while to comply.

Almost all communes have people who assist you in filling in tax forms at this tim of year. For free. Start with them.
