Leaving Belgium

Question

Hello all,
I'll shortly be leaving Belgium (1000 Brussels) for Canada. A definitive move, though I might return to live here a few years down the road. Here's my list of people to inform at this end, am I forgetting anybody/anything?
- Commune (de-register and surrender 'ID' card at latest possible moment)
- Tax (who? and how? by email, registered letter, in person...?)
- Bank
- Mutuelle
- Utilities (transfer contracts to new tenant who'll take over from me)

- Mobile phone contract?
- Make sure that you stop any direct debits, or standing orders on your bank account.
- If your bank allows you, it may be better to keep your bank account just in case you need to make any payments you forgot about, and if you are due a tax refund (in a year or so from now). It'll be easier to make and receive payments from a Belgian bank account, rather than from a Canadian one.
- Get your last tax form and the office where your taxes are done is noted on the front page. Call them and explain the situation, they'll tell you what you need to do.
