Prior Authorisation form for medical procedure
I need this PA form to lodge an application with my EC insurance for assistance with payment for a planned medical procedure
i believe its a set form - yet i cannot find it in the place i was told to look for it. There are many forms available that look similar - but none of them look like a PA form. My Q is would anyone in the same situation be able to tell me what is the heading on the form - at least that should speed things up.
i am not doing the application electronically.

If you don't have access to JSIS online, then simply fill in the ‘Request for Prior Authorisation’ form (pdf doc). Remember to attach the relevant original supporting documents. Then simply send everything to your Settlements Office (the address is on the form).
