Secondary Registration
I am considering secondary registration in Belgium
1) Are the procedures and check the same as for full residency?
2) Can I use it to get into the health system?
3) When will they start to get interested in taxes and income elsewhere?
(I will probably become a full resident soon, but most of my income is somewhere else)
There is no such thing as "secondary" registration.
If you have income in Belgium and elsewhere, seek out an accountant and get professional advice on your status.

If you reside in Belgium (= spend more nights a year there than in any other single country) then you have no choice but to register as a resident - and the process has to be started within 8 working days of the commencement of that residence. Once your registration has been confirmed you will become liable to personal taxation in Belgium including on income from elsewhere (although allowance will be made for tax properly paid elsewhere). If your registration is not confirmed for some reason, you will not be allowed to continue to live in Belgium.
